Medicare Part D Enrollment Event to Be Held in Burns Flat
The South Western Oklahoma Development Authority Area Agency on Aging (SWODA AAA) is hosting a Medicare Part D enrollment event along with the Oklahoma Senior Health Insurance Counseling Program (SHIP) on Tuesday, November 16, 2010, from 9:00 a.m. to 11:30 a.m. at the South Western Oklahoma Development Authority, 420 Sooner Dr., Burns Flat, OK. This enrollment event is for those on Medicare who need assistance finding a Medicare Part D plan or who have a Medicare Part D plan but may want a new plan.
The following information is required for those attending this event: the correct spelling of the beneficiary’s name, date of birth, beneficiary’s Medicare red, white and blue card, prescription medications, dosages of those medications and the number of times per day each medication is taken.
Time at the event will be limited, so call the SWODA AAA Information Assistant, Ada Vanderford, or the Planner, Staci Mauney, at 1-800-627-4882, to make an appointment or for questions about the event. Walk-ins are welcome, but this may extend the wait time.
